To be sold on the Russian market, a medical product has to be registered with the Ministry of Health. For that purpose a manufacturer must provide numerous documents translated into Russian, certified by a notary or court, and in the case of many countries also by the consular section of the Russian Embassy, the whole procedure being troublesome and expensive. Documents sometimes bear more than ten signatures and seals on both sides.
Technical, clinical, hygienic, and toxicological trials must be done by the Scientific Center for Expertise of Medical Products and other institutions for the purpose of registration. Clinical tests are done in two different clinical institutions for each product. Customs clearance also becomes more intricate with time, requiring voluminous paperwork.
There is information that bribes have been taken by civil servants of the Ministry of Health, Customs, and other structures affiliated with them. Foreign manufacturers are in some cases informed about it but have no choice but to pay, thus becoming embroiled in corrupt interactions.
Mediator firms offer assistance in registration, certification, and customs clearance of products. In return, exclusive distribution rights are sometimes requested from the manufacturer. In this way the mutuality principle of exclusive rights is violated: mediator firms make use of exclusive distributor's rights from several manufacturers at the same time. Additionally, a great number of customs brokers and brokerage firms are profiting from difficulties around customs clearance and registration.
The results of these complicated mechanisms of importation are increased prices for medical products on the internal market and delayed availability for patients.

Михаил Юрьевич, ну как же Ланцету разобраться, кто что берет и в каких случаях. Мой брат, довольно сносно говорящий по-русски, побывав на московской нейрохирургической конференции, совершенно запутался в механизмах "отката" и "проплачивания", не смог понять четких правил медицинского бизнеса. Попытки рассказать то, что он все-таки понял, департаменту маркетинга фирмы, которая его отправила как единственного русскоговорящего инженера, не увенчались успехом. Не верят.
